REGISTRATION

SECTION 1 – MEMBERSHIP INFORMATION

Print clearly or type all requested information.

Include your membership number, contact information, church, and denomination.

Indicate whether your husband will attend.

Mark if you are a first-time attendee.

Be sure to list the Convention City.

💰 SECTION 2 – FEES

Please check and calculate all applicable fees:

$100 – Convention Registration

$25 – Late Fee (if postmarked after April 30)

$75 – Husband’s Registration (if attending)

$10 – Executive Board Meeting (if eligible)

$500 – Life Membership (one-time fee, if applicable)

Scholarship Contributions (optional but encouraged)

Ticketed Events:

$60 – Legacy Luncheon

$60 – President’s Breakfast

$85 – Awards Gala Banquet

📝 Write each amount clearly and calculate your TOTAL at the bottom.

 

🎓 SECTION 3 – EDUCATION DEPARTMENT

Indicate if you are a graduation candidate.

Graduation participants must wear white clothes, white robe, and white shoes.

Please click the underline link (blue) and complete your Education Department registration. The blue link directs you to the official IAMWMW Education Department digital registration page.

All participants must use this online form to:

• View full course descriptions

• Select your required class - Select ONLY ONE class (25 slots per class – first come, first served).

• Secure your seat in your preferred module

• Ensure accurate and immediate registration processing

 

Alumni selecting Candle Craftsmanship must include the $10 supply fee in the total.
